Become a Housing Case Manager with Main Street Project in Winnipeg. Provide essential case management support to individuals facing homelessness and help them achieve their housing goals. As a key member of the Case Management team, you will support vulnerable individuals through housing-first and harm reduction approaches.
Your role will involve developing individualized care plans, assessing needs, and connecting participants with necessary resources. Building relationships and advocating for participants are central to this role and create positive community impact. Key Responsibilities:
