Be a vital part of patient care at Humber River Health as a Transplant Liaison Nurse within the Nephrology Program. This temporary full-time role focuses on enhancing kidney transplant access and patient experience. As part of Humber River Health, you will lead initiatives to empower patients during their kidney care journey.

The position requires a strong background in nephrology and a commitment to collaborate within the Ontario Renal Network. Your skills will directly support improved access to living kidney donation and facilitate patient readiness for transplant. Key Responsibilities:

- Establish relationships with Multi Organ Transplant Centers
- Ensure timely patient readiness and complete referral packages
- Track progress of transplant work-ups




- Collaborate with transplant centers to achieve quality care outcomes
- Educate patients and families about kidney transplant procedures Requirements:
- Current RN registration with the College of Nurses of Ontario
- Minimum two years of nephrology experience
- Experience in renal replacement therapies
- Understanding of Ontario Renal Network programs
- Robust communication and interpersonal skills Support patients through their kidney care journey and enhance transplant experiences at Humber River Health.
