Transform the lives of high-needs youth as an Integrated Care Coordinator for a Toronto-based non-profit. This full-time or part time role focuses on trauma-informed care and complex care navigation. In this vital position, you will support youth ages 12-17 recovering from sexual exploitation in a live-in treatment program.
Your responsibilities include conducting intake assessments, collaborating with interdisciplinary teams, and providing evidence-based clinical support. Your expertise in crisis management and trauma-informed care will be essential in facilitating therapeutic treatment sessions and ensuring compliance with critical regulations. Key Responsibilities:
- Connect with youth and families for intake assessments
