AllWellCare is hiring Licensed Practical Nurse (LPN), in home care with private clients where your compassionate patient care and nursing expertise will make a meaningful difference in the lives of those you serve. Responsibilities

- Deliver direct patient care by assisting with daily activities, vital sign monitoring, and health assessments.
- Administer medications accurately following prescribed orders and document all medication activities thoroughly
- Support registered nurses, health care aides and physicians with clinical procedures and treatment plans. Execute on the client’s care plan as outlined by the Case Manager.
- Maintain clear communication with patients, families, and healthcare team members to ensure coordinated care.
- Follow strict infection control protocols and uphold safety standards at all times.
- Record detailed patient information in medical records with precision and confidentiality
- Respond promptly to patient needs, providing comfort and reassurance during their healthcare journey.
- Provide care to clients such as medication administration, vitals,



monitoring client’s health as well as ADL’s if required. Tasks may include wound care, delegation of complex tasks to Health Care Aides.
- Looking for all visit times; day, evening, night, weekdays and weekends shifts.

Requirements: Minimum 1 year previous experience an asset.

Proficient in English, Hindi, Punjabi and other languages are an asset.

Proof of LPN Certification.

Current TB Screen and Criminal Record Check within the year.
