Southern Health-Santé Sud in Manitoba is seeking a Social Worker - Palliative Care to provide consultation, assessment, treatment, and counseling for individuals and families facing terminal illness, and to support grief and bereavement. The role works within an inter-disciplinary team delivering person-centered care, and includes education for patients, families, staff and community groups, with opportunities for program evaluation and research where possible.
