In this contract position, you'll work within a team-oriented multidisciplinary team to deliver medical interventions and support services. Your expertise will aid participants in achieving stability and wellness, while emphasizing trauma-informed and person-centered care principles. Key Responsibilities:

- Provide assertive outreach services to individuals with significant mental health issues
- Conduct assessments and plan supportive care for program participants




- Collaborate with the Housing First Medical Clinic for care coordination
- Complete monthly assessments and necessary documentation
- Assist participants with accessing health resources and advocacy Requirements:
- Bachelor's degree in Nursing
- Active registration with CRNA
- Minimum two years of nursing experience
- Competency in venipuncture and specimen handling
