We support over 4,000 people per year through a range of services including case management, supportive housing and adult day programs.About the PositionThe Nurse Supervisor, Clinical Best Practice provides onsite clinical oversight of Cota’s medication program, participates in new tenant intake, discusses medication changes, medication reconciliation, administration and responds to medication errors or high-risk scenarios. They serve as the community-based lead for service user engagement and implement a strategy aligned with Accreditation Canada best practices. They also participate in the Performance, Quality & Risk Management leadership team, contributing to client and staff safety initiatives, quality improvement and accreditation readiness.Primary ResponsibilitiesMedication Program OversightConduct weekly onsite medication audits across all supportive housing programsLiaise with pharmacies, prescribers,
care teams and external stakeholdersAct as co-chair for Cota’s Medication CommitteeLead accreditation preparation for the medication management standardRespond to medication incidents within the same or next dayLead investigations and implement corrective actions in a timely mannerProvide onsite outbreak responseTraining and EducationProvide clinical coaching, mentorship and support to frontline staffLead medication training and facilitate other trainings such as first aid and harm reductionImprove staff competency and confidence in medication managementDevelop and implement a service user engagement strategySupport service user engagement initiatives and tenant participation activitiesIncrease engagement at both site-based and community programsSupport broader community-based programs as neededOther ResponsibilitiesParticipate in accreditation preparation and quality improvement activitiesCollaborate on policy development and updatesEnsure timely and accurate documentation in electronic health records, including eMARComply with all Health and Safety policies and proceduresLead as part of Cota’s Clinical Leadership Team and participate in Leadership Forum and committeesWork cooperatively with service users, team members, management, other providers and stakeholdersMaintain confidentiality of service user,
