As a Quality Improvement Facilitator , you will focus on Home and Community Based Services (HCBS) provider quality and will evaluate HCBS waiver, habilitation, CNRS providers, and self-directed services to ensure quality and compliance with state and federal rules and laws, and industry best practice standards. Candidates outside these locations will not be considered. Conduct comprehensive quality evaluations of HCBS waiver, Habilitation, CNRS providers, and self-directed services Assess provider compliance with state and federal regulations, policies, and procedures Provide technical assistance and guidance to providers on HCBS topics Analyze data and prepare reports on provider performance and compliance Facilitate training sessions and educational workshops for providers Collaborate with stakeholders to implement quality improvement initiatives Monitor provider progress in meeting quality standards Bachelor’s degree in healthcare administration, social work, public health, or related field Minimum 3-5 years’ experience in HCBS or quality improvement Knowledge of state and federal HCBS regulations and requirements Understanding of quality assurance principles and methodologies Proficiency in Microsoft Office Suite and able to use various types of technology Valid driver’s license and ability to travel within assigned region Knowledge of person-centered planning principles Background in disability services or long-term care Training or teaching experience Knowledge of continuous quality improvement methodologies Experience with data analysis and reporting tools Experience working remotely Telligen is one of the most respected population health management organizations in the country.
