Responsibilities

- Deliver comprehensive patient care by assessing needs, developing care plans, and implementing nursing interventions with precision and compassion
- Administer medications and treatments accurately in accordance with physician orders and established protocols
- Monitor patient conditions vigilantly, recognizing changes that require prompt intervention or escalation
- Assist with diagnostic procedures such as phlebotomy and laboratory specimen collection to support accurate diagnosis and treatment
- Maintain detailed medical records using electronic health record systems like Epic or Cerner to ensure seamless communication among healthcare providers
- Support dementia, senior, hospice, pediatric, and assisted living patients by providing specialized care tailored to their unique needs
- Educate patients and families about health management,

Requirements

- Valid registration as a Practical Nurse with the applicable licensing body
- Proven experience in nursing home settings, hospitals, or acute care environments demonstrating strong patient care skills
- Knowledge of medical terminology, medication administration procedures, and infection control practices
- Experience working with diverse populations including seniors, children, hospice patients, or those requiring dementia or pain management care
- Familiarity with electronic health record platforms such as Epic or Cerner; medical office experience is a plus
- Ability to perform laboratory procedures like phlebotomy and handle laboratory specimens accurately
- Excellent communication skills to collaborate effectively with healthcare teams and educate patients effectively
- Strong organizational skills for case management tasks and maintaining detailed documentation
