Job Summary

We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our dynamic healthcare team. Duties

- Deliver comprehensive nursing care to patients in various settings, including nursing homes, hospitals, and home care environments
- Administer medications accurately and safely in accordance with physician orders and established protocols
- Monitor patient health status by performing assessments, vital sign checks, and documenting findings diligently
- Assist with medical procedures such as phlebotomy (blood draws), wound care, and pain management techniques
- Collaborate with interdisciplinary teams to develop and update individualized care plans tailored to each patient’s needs
- Manage case documentation using electronic health record systems like Epic or Cerner for seamless communication and record-keeping
- Provide dementia care,



hospice support, and senior or assisted living care with empathy and professionalism

Skills

- Proven experience in nursing home, hospital, or acute care environments with a strong understanding of medical terminology
- Skilled in medication administration, patient care routines, and laboratory procedures such as phlebotomy
- Familiarity with electronic health record platforms including Epic and Cerner for efficient documentation
- Knowledge of specialized areas like dementia care, hospice support, pediatrics, and pain management
- Excellent communication skills to effectively collaborate with healthcare teams and educate patients and families
- Ability to handle emergency situations calmly while providing compassionate patient-centered care
- Experience working with diverse populations including children, seniors, and individuals requiring hospice or assisted living services
