Registered Nurses (RN)
and
Registered Practical Nurses (RPN)
for our newly established integrated care teams at Are you looking for an opportunity to flex your nursing skills while being part of a new care model in your community?
Full time and part time guaranteed hours are available with a mix of day and afternoon shifts.

Job Description You will be responsible for providing high-quality nursing care to patients in the community. You have clinical skills and knowledge of wound management, infusion therapy, palliative care, and chronic disease management.
Provide patient-focused, safe, holistic nursing care and work collaboratively with other members of the health care team
Ensure that nursing care provided to our patients is based on current and best practices to achieve successful outcomes
Continually assess, anticipate, identify and respond to current and changing needs of our patients
Travel within your region between patients’ homes
Qualifications
Minimum two years nursing experience (Community Nursing experience is ideal)
Registered and in good standing with the College of Nurses of Ontario (CNO)
Ability to work independently in the community
Proficiency in English, both verbal and written required (additional languages an asset)
Valid Ontario driver’s license and reliable vehicle
Experience with infusion therapy and wound care
Experience with palliative care and mental health considered an asset
Excellent patient assessment and teaching skills
