We work with our patients to improve their quality of life and support their desire to return to meaningful activities.Our patient’s success is optimized by self‑motivation, active participation and family support.Job DescriptionThe Occupational Therapist is accountable for the following:Assessment of patients with functional impairment due to a variety of underlying medical conditions including but not limited to cardio‑pulmonary, musculoskeletal and neurological systemsCo‑creating of goals with the patient and familyPlanning and implementation of treatment plans to improve function and participation in activities of daily livingParticipate in patient discharge planningDocument patient assessment and treatment in the health recordCollaboration with the interprofessional healthcare team,
