You will liaise with community agencies, support caregivers, and participate in educational services, program development, and policy recommendations. and procedures.Promote client autonomy and ensure participation in planning and decision making.Assist in developing educational programs for home health care staff and community agencies on psychosocial health issues.Prepare statistics for workload measurement and maintain written clinical records.Perform other related duties as assigned.QualificationsMaster's Degree in Social Work from an approved school of Social Work.Two (2) years of recent related experience in a complex health care environment (e.G., community or home health setting).Current full registration with the BC College of Social Workers.Valid BC Driver's License and access to a personal vehicle for business‑related purposes.Applicants with a Bachelor of Social Work (BSW) degree may be considered if accompanied by an equivalent combination of education, training, and experience.
