As a Licensed Practical Nurse, you will report to the Director of Health & Wellness, ensuring that nursing care is goal-directed and aligned with provincial standards. Responsibilities include completing care assessments, administering medications, and facilitating effective communication across interdisciplinary teams to lead to successful patient outcomes.
Key Responsibilities:
• Evaluate care/service plans and implement necessary changes • Maintain thorough documentation of residents' care • Administer medications and support daily healthcare needs • Assist in care conference scheduling and participation • Respond to concerns from residents and families
Requirements: • Active registration as an LPN in the Province/State • Experience in a senior care setting is preferable • Strong communication, coordination, and critical thinking skills • Basic Cardiac Life Support certification is a must • Commitment to obtaining a Vulnerable Sector Check
