This full-time role combines advanced nursing skills within a supportive team. Candidates must have registration with the BC College of Nurses and Midwives and at least two years of relevant experience, focusing on primary care, addictions, or mental health. Responsibilities involve client assessments, care plan development, and collaboration in case management.
Key Responsibilities: • Conduct assessments and engage current clients/patients • Document care plans and interventions • Facilitate interdisciplinary team meetings • Support harm reduction and addiction assessments • Educate clients on health management strategies
Requirements: • Registration with BCCNM as an RN or RPN • 2 years' experience with diverse populations • Completion of HIV Testing course • Knowledge of trauma-informed care principles • Ability to perform venipuncture and CPR procedures
