Applicants are encouraged to self-declare in their application.Position SummaryFunctioning as part of the coordinated service delivery team, the Outreach Worker works proactively with current and existing clients, offering a range of support and recovery services, within an existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate and provides services in a non-judgmental and culturally appropriate manner, helping clients to build new skills and achieve their goals.This Program works within an Assertive Community Treatment Model and utilizes the Housing First Principles from a social justice orientation which includes harm reduction, pro-choice, social determinants of health, human rights, social development, population health, health equality and trauma informed,
