Conduct in-home assessments including functional evaluations
Develop individualized, evidence-based treatment plans

Requirements:
Registered and in positive standing with COTO
Experience in home and community care
Solid skills in functional assessments and safety evaluations
Valid driver’s license and reliable transportation
Excellent documentation skills in clinical settings
