Join Integrated Services as an Elder Support Worker, focusing on the well-being of Elders in Cold Lake First Nations. Engage with community resources and promote Elder independence and connection. In this full time role, Integrated Services is looking for an Elder Support Worker to provide essential, relationship-based support for Elders.

You'll perform wellness check-ins and home visits while coordinating access to vital services. Building robust relationships and ensuring Elders' safety are key components of this position. Key Responsibilities:
Conduct regular wellness check-ins with Elders
Assist Elders in accessing health and social resources
Provide referrals and follow-up support
Coordinate community and cultural activities for Elders
Work closely with families and service providers

Requirements:
Diploma in Social Work or related fields preferred
Experience in community support or wellness programs
Solid advocacy and relationship-building abilities
Familiarity with Cold Lake First Nations community
Criminal Record Check and Driver’s License required
