Make a difference in mental health with weCHC as a Case Worker specializing in Black youth. This full time role supports comprehensive care strategies in Windsor's community health sector. The AMANI program at Windsor Essex Community Health Centre seeks a confident Case Worker to join their team.
This position focuses on youth aged 12 to 29 facing co-morbid mental illness and substance use challenges. You will provide tailored, culturally responsive support that reflects the resiliency and strength of Black communities. Key Responsibilities:
Implement client-centered case management strategies
Monitor and evaluate program effectiveness
Document client progress and collaborate in case conferences
Advocate for clients with external community resources
Ensure culturally responsive care in all interactions
Requirements:
Experience in counseling or therapy in community settings
Background working with Black youth
MSW or BSW with relevant clinical experience
Solid communication and interpersonal skills
Valid driver's license for transportation needs
Help empower youth through holistic social work approaches tailored to their unique experiences and needs.
