By bringing together several of the top home health agencies in the state, we have become one of New York’s leading providers with strategically placed offices that serve hundreds of clients every day.Qualifications:Must have NYS certification for PCA & HHA'sMust be 18 years oldHigh school diploma or equivalentPreferred six (6) months experienceMeets health state requirementsCommitted to health & safetyExcellent communication and people skillsPositive and compassionate attitudeResponsibilitiesBy following the Care Plan that has been completed by the Nursing Supervisor, the Home Health Aide provides assistance with the following:Monitoring patient condition by observing physical and mental condition, intake and output and reporting changes to nursing supervisors.Maintaining a healthful, safe environment.Providing nutritional and environmental support.Assisting with personal hygiene, toileting, dressing, walking and feeding the client.Positioning the client as per care plan.Assists patients by providing personal services, such as bathing, dressing and groomingSupports patient by providing housekeeping and laundry services, food shopping and running necessary errands as per care plan.Preparing and serving meals and snacks as directed by care planAssisting with certain treatments and therapy as ordered by the physician and approved by the nursing supervisor.Reminding the client to take medicationsAssist patient with shopping efficiently utilizing patient’s money and observing cost‑saving techniques.Obtaining patient’s vitals, understanding values not within normal limits,
