We are looking for a reliable Care Partner for a patient transitioning from clinic-based dialysis to home care. This role emphasizes personalized attention with consistent support, enabling strong rapport-building. You'll assist with the NxStage or Tablo machine setup, maintain a clean space, and monitor the patient's treatment schedule.
Key Responsibilities:
• Assist with dialysis treatment setup using NxStage or Tablo
• Maintain a clean, organized, and safe treatment setting
• Provide companionship and emotional support to the patient




• Help the patient adhere to treatment schedules
• Document shift notes and monitor safety concerns
Requirements:
• Minimum of 1 year dialysis experience (clinic or home)
• Familiarity with home dialysis systems preferred
• Open to in-home support, willing to undergo training
• Strong communication skills with clinical teams
• Must pass a background check
