Shape the future of community health at CommunitiCare Health as a Permanent Family Physician. This role emphasizes collaboration, health promotion, and specialized care, including Low-Risk Obstetrics. You will play a pivotal role in an inter-qualified team, creating access for clients facing multiple barriers and complex health challenges.
Your work will include patient care, community outreach, and participating in quality improvement projects. With a focus on comprehensive healthcare delivery, you will have a significant impact on diverse populations. Key Responsibilities:
Provide primary medical care and act on urgent lab results
Foster collaboration with nursing and healthcare teams
Implement and evaluate community health initiatives
Maintain thorough documentation of client interactions
Contribute to inter-qualified research initiatives Requirements:
Medical degree and valid Ontario medical license
Membership in the College of Family Physicians of Canada
Experience working with vulnerable populations is preferred
Robust clinical acumen and problem-solving abilities
