Shape integrated care services with Rideau Community Health Services as their Director of Integrated Care. Lead program development, quality improvement, financial oversight, and interdisciplinary collaboration across multiple communities.

As a key member of the Senior Leadership Team, this role encompasses strategic and operational guidance within RCHS. You’ll implement initiatives that improve health promotion and service integration while advocating for primary care services. Your leadership will empower teams and drive quality in service delivery across Leeds, Grenville, and Lanark counties.

Key Responsibilities:
• Lead the planning and evaluation of integrated care services
• Oversee development of workplans aligning with strategic priorities




• Advocate for quality improvements in Primary Care Services
• Build partnerships with community and funders
• Support interdisciplinary teamwork and communication

Requirements:
• Master’s degree in health administration or related field preferred
• Leadership experience in health and social services
• Valid driver’s license and reliable vehicle
• Strong understanding of funding data requirements
• Ability to work flexible schedule according to program needs
