Provides home‑based supports, outreach, system navigation, and culturally grounded health promotion. HHAs focus on personal care and daily living supports; CHRs serve as liaisons between providers and community members to support access and education. What You’ll Do:
Home visits, vitals, and basic supports within scope.
Accompany clients to appointments; navigation and advocacy.
Health promotion, materials, and reminders.
Community outreach and program support.
Prerequisites
Grade 12 diploma — Regular Grade 12, Adult 12, or GED.
