Join the Winnipeg Regional Health Authority as a Clinical Coordinator within the Health Outreach & Community Supports team. This in-person position is focused on improving health outcomes for vulnerable populations. In this role, you will respond to requests for assistance from at-risk groups and their supporting agencies.

The Clinical

Coordinator will utilize extensive mental health expertise to develop service plans that address client needs and increase organizational capacity. Establishing solid relationships with community staff, health agencies, and management is vital to your success. Key Responsibilities:
Assist in coordinating health and social service plans
Evaluate and assess needs in partnership with community agencies




Serve as a direct contact for service-related inquiries
Ensure compliance with documentation and consent standards
Enhance capacity for both individuals and agencies Requirements:
Four years of related community experience required
Minimum two years in clinical coordination
Relevant health/human services degree essential
Valid driver’s license and access to a vehicle
Certification in Non-violent Crisis Intervention is required Drive effective health service delivery as a Clinical Coordinator focused on community outreach.
