As part of the Medicine Department, the Nurse Practitioner works closely with the Hospitalist group and other healthcare professionals. You will develop care plans for patients in the transitional unit and manage discharge processes for patients waiting for Long Term Care. This role demands advanced nursing knowledge and the ability to autonomously diagnose and prescribe medications.

Key Responsibilities:
• Collaborate with the interdisciplinary team for patient care
• Develop and implement plans for transitional care unit patients




• Order and interpret diagnostic tests and prescribe medications
• Lead discharge planning processes effectively
• Promote patient safety and adherence to health policies

Requirements:
• Master’s Degree in Nursing required
• Registration with the College of Nurses of Ontario in the Extended Class
• Adult or Primary Health Care Specialty Certificate
• Experience in acute in-patient care preferred
• Current BLS certificate and valid driver's license
