This role involves triage and outreach for individuals facing mental health and substance use challenges. As part of an interdisciplinary team, you'll assess needs, provide counseling, and facilitate access to healthcare services. Your contributions will help reduce client reliance on acute care through proactive interventions.
Key Responsibilities:
• Conduct mental and physical assessments for clients
• Provide crisis intervention and support services
• Collaborate on integrated care plans with a team
• Facilitate client movement between care services
• Assist clients in attending critical services and appointments
Requirements:
• Registered Nurse or Registered Psychiatric Nurse license
• Two years’ experience in mental health and substance use
• Valid BC Driver's License and personal vehicle
• Knowledge of crisis intervention and psychopharmacology
• Ability to work independently and as part of a team
