Key Responsibilities:
• Participate in scheduled community visits for patient care
• Assess patients’ needs using appropriate techniques




• Execute and monitor customized care plans
• Build rapport and trust with patients
• Maintain and update professional patient records

Requirements:
• Registration as a Registered Practical Nurse with CNO
• Valid driver’s license and vehicle access
• Experience in community care is advantageous
• Flexibility for travel to patient homes
• Willingness for on-call coverage and after hours
