Key Responsibilities:
• Conduct home safety assessments and equipment prescriptions
• Develop fall prevention strategies tailored to individual needs




• Implement rehabilitation plans for stroke and chronic conditions
• Supervise occupational therapy assistants as required
• Educate clients on available community resources

Requirements:
• Registration in good standing with COTO
• Solid communication and documentation skills
• Experience in community practice is an asset
• Valid G or G2 driver’s license for travel
• Proficient in MS Office Suite and clinical software
