The Alex, a community-based not-for-profit, is looking for a skilled Registered Nurse for its Pathways Assertive Outreach program. In this contract position, you'll work within a team-oriented multidisciplinary team to deliver medical interventions and support services. Your expertise will aid participants in achieving stability and wellness, while emphasizing trauma-informed and person-centered care principles.

Key Responsibilities:
• Provide assertive outreach services to individuals with significant mental health issues
• Conduct assessments and plan supportive care for program participants




• Collaborate with the Housing First Medical Clinic for care coordination
• Complete monthly assessments and necessary documentation
• Assist participants with accessing health resources and advocacy

Requirements:
• Bachelor's degree in Nursing
• Active registration with CRNA
• Minimum two years of nursing experience
• Competency in venipuncture and specimen handling
• Valid driver’s license and personal vehicle
