What we are looking for: The Virtual Safety Monitoring program is dedicated to remotely monitoring and ensuring the safety of patients. The VSM has enhanced patient care, prioritizing patient safety and well-being by addressing gaps in traditional healthcare systems, ensuring consistent and attentive care at all times.
Reporting to the Patient Care Manager, Access and Flow, as a member of the Access and Flow Command Center team, the Virtual Safety Monitoring Technician plays a vital role in providing support and assistance to patients through remote communication channels by providing continuous observation & monitoring to patients with safety concerns.
This role requires robust communication, technological proficiency,



keen observation and critical-thinking skills, as well as the ability to remain calm, focused and responsive in a high-pressure environment.
In this role, you will: Monitor patients remotely using advanced telemonitoring technology to promote patient safety and well-being.
Maintain continuous observation of patients with identified safety concerns and recognize changes in behaviour or condition.
Communicate with patients through remote channels, providing reassurance, support and assistance as required.
Communicate changes in patient behaviour or condition to the appropriate clinical teams and elevate concerns promptly when required.
Work closely with interprofessional healthcare teams to support timely response, de-escalation and resolution of safety concerns.
Complete accurate and timely documentation of patient observations and interactions.
Support the assessment and triage of patients for suitability for the Virtual Safety Monitoring program.
Participate in continuous improvement initiatives that support program development, innovation and patient safety.
Manage multiple priorities effectively while monitoring up to 12 patients simultaneously.
Use Microsoft Office and other technology platforms confidently and efficiently.

Qualifications: Secondary School Diploma required.
Bachelor's degree in health or related areas is preferred.
2-4 years of previous experience in a customer service or front-facing position required.
1-3 years of previous experience working in a healthcare environment preferred.
Previous experience with Meditech is considered an asset.
Training or certification in Gentle Persuasion Approach (GPA), Dementia Ability Methods, P.I.E.C.E.S. or Crisis Prevention Intervention is
