As a Clinical Coach, you will drive practice improvements in palliative care. Your expertise will be pivotal in training healthcare professionals and implementing change aligned with the Ontario Palliative Care Network standards. You will collaborate with health partners to facilitate quality care and education, ensuring priority organizations have the necessary resources.
Key Responsibilities:
• Educate healthcare providers on palliative care delivery
• Support quality improvement activities in various settings
• Mentor clinical staff in long-term and community care
• Collect and report data as needed for provincial alignment
• Facilitate partnerships with Indigenous organizations
Requirements:
• Undergraduate degree in health or related field
• Current health qualified registration in good standing
• Minimum five years' palliative care experience
• Previous clinical coaching and supervision experience
• Strong communication and relationship-building skills
