What you will do - Conduct Care Act assessments and reviews for adults transitioning from hospital to community.

- Collaborate with hospital teams to ensure safe and timely discharges.
- Craft person-centred care plans that encourage recovery and maximised independence.
- Evaluate eligibility for adult social care services and propose suitable reablement options.
- Liaise with multidisciplinary teams including occupational therapists, nurses, and community organisations to deliver comprehensive care.
- Facilitate access to reablement services, helping individuals regain their independence.
- Analyse and manage risks related to discharge, ensuring safe and supportive transitions.
- Acknowledge and act on adult safeguarding concerns, promoting safe environments.
- Apply relevant legislation and make informed decisions with professional integrity.
- Engage families and carers, incorporating their insights into care plans.
- Provide detailed guidance to individuals and families about service options.
- Coordinate responses for adults with complex needs, ensuring a multi-agency approach.
- Maintain detailed and accurate case records, producing professional documentation as required.
- Manage your caseload effectively, addressing cases based on risk and urgency.
- Actively participate in meetings and discussions, contributing to discharge planning.
