Become an integral part of community health as a Community Health Nurse working in Thunder Bay, ON. This dynamic role focuses on enhancing public health and in-home nursing services. Reporting to the HCC Nurse Manager, you will be responsible for the holistic health of clients in KO communities by developing and implementing nursing programs.
Your responsibilities include assessing patient needs, coordinating care with Community Personal Support Workers, and ensuring public health services are culturally appropriate and effective. Key Responsibilities:
- Implement community-focused nursing services for clients
- Create goal-oriented, client-centered care plans
- Oversee Community PSWs to ensure high-quality nursing delivery
- Build relationships with local healthcare professionals
- Contribute to health reports and program evaluations Requirements:
- Valid RN or RPN license with the College of Nurses of Ontario
- Minimum 2 years of related nursing experience
- Advanced Foot Care certification preferred
- Robust communication skills in English; Oji-Cree or Ojibway a plus
- Willingness to travel to remote communities Utilize your nursing acumen to provide impactful health services to First Nations families in Thunder Bay.
