Through UHS and its affiliated corporations, the organization provides a continuum of services and supports designed to promote healthy aging, independence, dignity, and quality of life for older adults.

UHS operates a long-term care home, seniors’ housing, a seniors’ active living centre, an adult day program, and geriatric outreach services. Together, these programs provide seniors with a range of care, housing, wellness, and community-based supports tailored to their individual needs.

Position The UHS Geriatric Outreach Team provides specialized, community-based support to older adults experiencing complex health, functional, cognitive, and psychosocial needs. The team works collaboratively with seniors, families, caregivers, primary care providers, and community partners to provide comprehensive assessment, consultation, education, and care coordination.

The Geriatric Outreach

Team also supports families, caregivers, and community providers by providing education, guidance, and clinical consultation.

Through an advanced practice role, the Geriatric Outreach Team Clinician brings specialized geriatric knowledge and expertise to the community, supporting the early identification and management of complex needs and helping to connect seniors with appropriate health and social services. This collaborative approach strengthens continuity of care, supports informed decision-making, and helps seniors remain safe, independent, and engaged in their communities for as long as possible.

Essential Job Functions

Clinical Responsibilities:

· Conduct Comprehensive Geriatric Assessments (CGA)of an older adult including evaluation of medical conditions, cognitive status, functional abilities (ADLs/IADLs), mental health, and social supports

· Apply advanced clinical knowledge, critical thinking, and clinical reasoning to assess, identify and manage common geriatric syndromes (e.g., falls, frailty, polypharmacy, incontinence, multimorbidity) and determining appropriate interventions, referrals, and escalation of care.

· Provide short-term clinical follow-up and develop individualized, patient-centered care plans that promote safety, independence, and quality of life

· Develop evidence-informed clinical recommendations and individualized care plans in collaboration with clients, families, caregivers, and healthcare providers.

Risk Assessment and Safety Planning:

· Assess risks related to falls, cognitive impairment, self-neglect,



and caregiver burnout and other factors affecting safety, function, and quality of life.

· Collaborate with clients and families to develop and implement safety plans when necessary

Care Coordination and System Navigation:

· Liaise with primary care providers, specialists, hospital teams, and community agencies

· Facilitate referrals to appropriate services (e.g., home care, community supports)

· Connect clients and families with specialized services and community resources, while identifying service gaps and advocating for appropriate supports.

Interdisciplinary Collaboration:

· Work within an interprofessional team (e.g., nurses, social workers, occupational therapists, physiotherapists, physicians)

· Communicate clinical findings, recommendations, and follow-up plans clearly and effectively to clients, families, referral sources, and service providers.

· Participate in weekly rounds presenting cases to consulting Physician

Other Related Functions

· Provide clinical consultation and support to intake and triage, including assessment of referrals and determination of appropriate service pathways.

· Advocate for clients and families, supporting informed decision-making, dignity, safety, independence, and quality of life.

· Maintain strong collaborative relationships with community agencies and healthcare partners and contribute to community education and awareness of Geriatric Outreach Team services.

· Contribute to the development and maintenance of clinical resources, manuals, and educational materials.

· Maintain current knowledge of geriatric best practices, community resources, and emerging service needs.

· Practice in accordance with organizational policies, professional standards, and requirements for the safety of clients, families, colleagues, and the work environment.

Qualifications

Required Knowledge, Skills & Abilities:

- Valid driver’s license and access to a reliable vehicle.
- Member in good standing with the respective professional College.
- Demonstrated experience in community-based allied health, including in-home assessment and service delivery; community experience with older adults is an asset.
- Advanced clinical skill set, including comprehensive assessment, clinical reasoning, critical thinking, risk identification, care planning, and independent clinical decision-making within professional scope, preferably with older adults/geriatric community.
- Strong knowledge and application of geriatric principles and Comprehensive Geriatric Assessment (CGA) across medical, cognitive, functional, psychological, and social domains.




- Ability to assess and manage complex, multifactorial needs of older adults in the community and coordinate appropriate interventions and services.
- Experience working within an interprofessional and community-based care environment, with the ability to collaborate effectively across disciplines and sectors.
- Experience in long-term care, seniors’ services, or other geriatric settings is an asset.
- Strong organizational and time-management skills, with the ability to independently manage schedules, competing priorities, and clinical responsibilities.
- Demonstrated ability to exercise sound clinical judgment, discretion, tact, and diplomacy in complex situations.
- Excellent written and verbal communication skills, with the ability to communicate clinical information effectively to clients, families, caregivers, healthcare providers, and community partners.
- Strong interpersonal skills and an empathetic, client-centred approach, with a genuine commitment to promoting the health, dignity, independence, and well-being of older adults.
