Position Summary:
The Nurse Practitioner (NP) is a key member of the integrated HART Hub interdisciplinary team, providing primary health care services to individuals experiencing homelessness, housing instability, mental health concerns, and/or substance use challenges.
Reporting to the Director of Nursing, the NP delivers clinic-based, outreach, and virtual care within a collaborative, multi agency environment. The role focuses on providing episodic and ongoing primary care to individuals unattached to a primary care provider and face barriers accessing traditional health services.
The NP works closely with Hart Hub partners including the Bridge RN, Community Paramedics and Outreach Teams, Addictions Workers, guest services staff, and Transitional housing programs. In addition, they will provide their clinical expertise and leadership amongst the team and work collaboratively to coordinate care, improve health outcomes,
and support clients in achieving stability and recovery.
Minimum Qualifications & Experience:
- Current registration in the Extended Class (Nurse Practitioner) and in good standing with the College of Nurses of Ontario.
- 2-3 years nursing experience in a community setting, public health setting, and/or primary care setting.
- Street or outreach nursing experience is an asset.
- Training or certification in safe prescribing practices and substance use disorders management preferred; willingness to obtain relevant education upon hire required if not already held.
- Experience supporting individuals experiencing homelessness, substance use, mental health concerns, and other complex social needs.
- Strong clinical judgment, problem-solving, organizational, communication, and interpersonal skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Understanding of anti-oppression practice, trauma informed care, and recovery-oriented frameworks.
- Comfort working in flexible, non-traditional care environments, with diverse populations.
- Valid Ontario driver’s license, reliable vehicle, proof of insurance satisfactory driving record and ability to travel throughout Hastings and Prince Edward Counties and the Quinte region.
- Basic proficiency in computer skills; including EMR and Microsoft Office preferred.
- Ability to work flexible hours, weekends and evenings may be required.
- Satisfactory criminal record check (CRC)
and Vulnerable Sector screening prior to hire.
