Home Horizon Georgian Triangle Residential Services is a not-for-profit registered charity dedicated to transform the lives of youth at risk of homelessness by providing them with the opportunity to rebuild their lives and achieve their full potential. These youth are in the process of transitioning into adulthood, and due to challenging circumstances, may not yet have acquired the health, personal, social, and life skills that make independent living possible.

At the Barbara Weider House (BWH), we take care of youth’s immediate needs and offer them the chance to change their lives by providing stable housing and support. BWH bridges the identified critical gap between emergency shelter and independent living for youth aged 16-24. The Barbara Weider House offers a place of safety and hope for homeless youth.

Working as a team, two Residential Case Workers will directly report to and work alongside the BWH Program Manager, adhering to their direction, to deliver client support. Supportive direction will be developed with a team approach with the Executive Director having ultimate discretion.

The Residential Case

Worker is responsible for the direct implementation of Home Horizon’s philosophy, policies and procedures as they pertain to client centered care.

Worker supports BWH by fulfilling the outlined responsibilities within the Youth Worker role, while also providing direct and ongoing support to a predetermined roster of clients. This role is designed to ensure all youth are supported effectively and their physical, emotional, social and cognitive needs are considered.

Some key aspects of this role include, but are not limited to: assisting in the day to day operations of BWH, carrying out tasks directly delegated by the Program Manager, utilizing a holistic and strength-based approach in providing client focused care, supporting program creation, implementation & assessment, connecting youth with appropriate external supports and/or services, maintaining facility and security needs as well as community relations.

Key Role Responsibilities (but not limited to):

- Participates in weekly case management meetings with BWH Program Manager
- Develop and complete daily case notes pertaining to all matters of BWH and rostered clients
- Conduct grocery and household supplies inventory check
- Maintains that the budgetary requests and expectations are adhered to




- Adheres and implements household cleaning schedule and makes recommendations as needed
- Assist in client case management as delegated by the Program Manager. Assess, identify and advocate for up to a roster of five (5) individuals.
- Assist in connecting youth to external services and resources, booking appointments, locating and/or providing accessible transportation as well as follow-up.
- Update and maintain client calendar (appointments, work schedules, vacations/outings, birthdays, graduations, etc)
- In collaboration with the Program Manager assist to conduct room inspections with supporting Youth Worker staff, complete appropriate documentation and follow up with Program Manager regarding findings (areas needing attention, client progress, confiscations, etc)
- Provide support in organizing and facilitating house meetings for internal clients as delegated by the Program Manager.
- Assist in providing in-the-moment intervention, prevention and coping strategies for youth
- Provide training and mentorship to new and existing frontline staff
- Maintains neutral and conscientious rank within the front-line staff team
- Uses tact and diplomacy to address and communicate issues to management and other stakeholders
- Maintain duties and participation of on-call responsibilities including seven days a month of on call phone support and call in coverage support
- Participate and contribute to team meetings
- Collect, organize and record client focused Life Skills statistics
- Assist in follow-up with youth regarding current program goals as delegated by the Program Manager
- Act as liaison between clients, youth workers, management and other stakeholders.
- Support and accept tasks delegated by the Program Manager.
- Understand and adhere to Home Horizons Policies, Procedures and Protocols
- Ensure information regarding service delivery is entered into the client information database by the end of each shift

Key Client Responsibilities (but not limited to):





- Maintains confidentiality at all times.
- Participate in mandatory training workshops and team meetings while promoting professional conduct.
- Meet regularly with Program Manager to assess, manage, design, and implement Plan of Care in respondence to the youth’s priority needs.
- Work collaboratively with youth and Program Manager to ensure client success in working towards their Plan of Care Goals.
- Communicate and document any change in the status or needs of individual clients while maintaining positive and effective relationships with clients and their families.
- Residential Case Workers will exercise tact and diplomacy when advocating for their clients.
- Provide youth with opportunities for community involvement as well as social activities.
- Promote and maintain a therapeutic environment for a diverse population of youth, aged 16-24, experiencing a range of social, emotional, behavioural and mental health difficulties.
- Develop therapeutic relationships with youth using various teaching, mentoring, and supportive skills.
- Household duties such as (but not limited to) light cleaning, assisting with day-to-day routines, providing therapeutic interventions, attending appointments, assisting with school,
- Administrative duties, which include reading and writing daily shift notes, incident reports, medication logs etc.
- Assist youth with practical resources including food, clothing, transportation, start-up kits, groceries, first aid, computer access/support, assistance filling out forms and applications
- Address reliable behaviour, mental health, emotional, and addictions issues through defusing/managing critical incidents, supportive relationships, and referrals to or community service providers, or calling emergency services if required
- Assist youth with accessing community support networks including: medical, legal, addictions, therapeutic, mental health, social recreational service providers
- Other duties that may arise as reasonable in the operation of a residential service environment

Job Types: Full-time, Permanent Pay: From $26.60 per hour
