- Provide one-to-one keyworking sessions and manage a caseload effectively to support individuals through their recovery journey.
- Implement harm reduction practices tailored to individual needs, offering guidance and support.
- Liaise with other professionals to create a comprehensive care plan for service users.
- Ensure all cases are documented in line with compliance and quality standards.
