We are committed to inclusive, equitable and culturally safe hiring practices.ResponsibilitiesDetermines appropriate nutrition intervention through physician/health team referral or risk screening for the assigned patient population; assesses status and determines medical/nutritional goals by reviewing charts, interviewing patients or family, or through discussion with the interdisciplinary team.Develops and implements a patient care plan based on information from multiple sources; documents care plan according to departmental standards.Evaluates the effectiveness of the intervention through observation and biochemical parameters; adjusts the care plan as required.Contributes to the Dietetic Internship Program by providing educational support, sessions to the interdisciplinary team, and community education; advocates for nutritionin health promotion and intervention.Acts as a resource in medical/nutritional interventions to the interdisciplinary team, allied health professionals, students and the community.Participates in the development of clinical guidelines, pathways, nutrition education materials,
and quality initiatives.Communicates the nutrition care plan to Food & Nutrition Services; participates in menu revisions, nutrition audits, and department initiatives; liaises between patientcare areas and the department.Participates in approved departmental and multidisciplinary research projects, collects and interprets data, and contributes to dietetic practice.Develops clinical indicators, maintains statistics, and contributes to program and departmental strategic planning for the area of practice.Performs other related duties as assigned.QualificationsEducation, Training and ExperienceBachelor of Science (BSc) in Nutrition from an approved educational institution and successful completion of an approved dietetic internship program.Current registration in the College of Dietitians of British Columbia.Valid BC Driver's License and access to a personal vehicle for business-related purposes, as required.Skills and AbilitiesKnowledge of current dietetic practice, standards, clinical protocols, and current developments and research.Ability to develop effective patient care plans and provide effective consultation.Strong communication skills, both verbal and written; ability to work with teams and independently.Ability to plan, implement, problem-solve, organize and prioritize work; ability to operate related software.Physical ability to perform the duties of the position.About Fraser HealthFraser Health is the heart of health care for over two million people in Metro Vancouver and the Fraser Valley, on the traditional lands of the Coast Salish and Nlaka’pamux Nations.
