PositionRegistered Nurse – Home Hemodialysis Program: Long-Term Care Satellites, Nephrology Program.Employment StatusPermanent Part-timeReporting RelationshipManager, Home Dialysis and Transitional Care Unit, Nephrology ProgramHourly Rate$41.15-$58.98Union GroupONAHours of WorkDays, Monday to Friday dayLocationChurch Site with travel requiredResponsibilitiesPerforming all aspects of patient care including care paths, patient education, home and community visits, transplant workup, and monitoring quality indicators to ensure treatment adequacy and patient safety is maintained for all primary patients and as assignedDemonstrating a commitment to ongoing career development within an evolving fieldProviding assessment, education, training and decision support for patients/family as it relates to independent dialysisCoordinating/facilitating patient appointments including clinic visits in collaboration with clerical team membersEngaging the interdisciplinary team in a collaborative approach to professional practice issues and acts in the role of Liaison between all members who participate in the care of the patient and their familyApplying relevant research to clinical practice using critical thinking and decision making; utilizing research/evidence based information to recommend nursing interventionsParticipating in and supporting CQI initiativesProviding a framework /smooth transition for patients who are involved in transitioning to the long-term care dialysis unitsCandidates will be required to visit the patients in their homes as part of the training and supportMeeting “on call” requirements both weekdays and weekendsRequired to work shifts as scheduledParticipating in Nephrology Program projects as assignedParticipating in rotational Home Support Nurse assignmentCommunicating and collaborating with home dialysis vendorCollecting, documenting,



and reporting clinical and non-clinical data into Meditech for the purpose of ORRS submissionQualificationsRegistered and in good standing with the College of Nurses of OntarioBLS Provider certified requiredCNeph Certification preferred. Required to complete CNeph certification or equivalent within 28 months from the hiring dateBaccalaureate degree preferredMinimum of 2 years recent nephrology experience with at least 18 months in hemodialysis requiredPrevious experience in pre-dialysis, transplantation, or home hemodialysis is an advantageExperience with ORN data collection preferredProficiency in using Microsoft office (Word, Outlook, Power Point and Excel) and MeditechDemonstrated strong ability to assess a patient’s readiness to learn and apply the principles of adult learning while teaching patients and familiesDemonstrated leadership skills in workload planningExperience in developing/implementing patient care algorithms and patient education toolsDemonstrated excellent patient assessment skillsDemonstrates competency in chosen profession; consistently demonstrates knowledge and expertise in professional practice and hospital-based standardsEngages in continues learning and reflective practiceDemonstrates being patient and consumer focused; makes a commitment to meet or exceed the needs of all internal and external customersExcellent communication and interpersonal skills requiredDemonstrated excellent organizational and planning skillsAbility to be flexible and effectively work in busy environment with changing priorities and deadlinesDemonstrated initiative and engagement in quality improvementDemonstrated ability to be self-directed and work independentlyDemonstrates fiscal responsibilityMust be able to complete the physical duties of the position, the “Registered Nurse” role description,
