Reporting to: Manager, Provincial Resource Centre – Organ Classification: Clinical Services Coordinator (0.75FTE) Worksite: 483BayStreet Schedule: Combination of weekday and weekend 12‑hour shifts.Trillium Gift of Life Network (TGLN), part of Ontario Health, coordinates organ and tissue donation and transplantation in Ontario. It is responsible for increasing public awareness of donation, educating health care professionals, and ensuring a safe and effective donation and transplant system.Working collaboratively with stakeholders, fostering organizational and individual growth, TGLN values honesty, trust, respect and compassion.ResponsibilitiesCase management of the donation process from referral to organ recoveryWorks collaboratively with donor hospitals, health care teams, transplant programs, tissue banks, air and land transportation carriers, labs and other TGLN staffCoordinates organ placement and any necessary testingResponsible for making critical decisions and ensuring that Health Canada regulations are followedFacilitates the logistics of organ recovery and arranges transportation for multiple recovery teams (surgeons, land and air transportation)Responds to calls from referring hospitals, conducts a preliminary assessment of the patient’s history and identifies all relevant donation optionsCollaborates with the attending health care team in developing strategies for integrating TGLN services into the end‑of‑life care plan for the patient and familyOffers health care team initial recommendations related to management of the process or of the patient’s physiologic status, as appropriate, until a Specialist,



Organ and Tissue Donation is involvedDocuments comprehensive and accurate case notes and enters key information into TGLN’s electronic chart and custom databaseActs as an expert resource in all aspects of organ and tissue donationPromotes and supports organ and tissue donation best practicesWorks 12‑hour rotating shifts in a busy 24‑/7 call centre environmentPerforms other duties as requiredQualifications & RequirementsNursing Diploma and/or Bachelor of Science in NursingCurrent RN registration with the College of Nurses of Ontario is requiredA minimum of one year of critical care nursing experience preferredOrgan donation or transplant nursing experience preferredAll day shifts (weekday and weekend) are required to work in office (subject to change)Duties require robust understanding of applied pathophysiology, medical diagnostic testing/imaging, and ability to interpret laboratory resultsMust be able to sort through large amounts of clinical information, understand and re‑communicate the information to internal and external stakeholdersConflict management skills and ability to influence through applying expertise/rational argumentsExperience in case management/coordinationExperience working in a fast‑paced time‑sensitive environmentAbility to problem‑solve in high‑pressure, time‑sensitive situationsExcellent communication skills – written and oralStrong computer skillsEmployment DetailsRate of Pay: $36.05 – $51.60 per hourEmployment Type: Permanent – Part time (0.75FTE)Employment Location: Toronto, OntarioAll applicants must be a resident of Ontario to be considered for roles at Ontario Health.Equal Employment OpportunityOntario Health encourages applications from candidates who are First Nations, Métis, Inuit, and urban Indigenous; Francophone; members of Black and racialized groups; 2SLGBTQIA+ communities; trans and nonbinary individuals; and people livingwith disabilities.
