Key Responsibilities:

- Provide on-site patient care using a harm reduction approach




- Focus on HIV, Hepatitis C, and chronic condition management
- Conduct assessments, diagnoses, and treatment planning
- Prescribe and manage medications
- Educate patients on prevention and wellness
- Collaborate with physicians and interdisciplinary teams
- Follow CNO standards and privacy regulations
- Engage local health and community organizations

Qualifications:

- Registered NP (Adult or Primary Health Care) with active CNO license
- Masters degree (MN-NP or MScN)
- Preferred: experience in harm reduction, HIV/Hep C, or community health
- Strong clinical decision-making and patient communication skills
- Proficient in EMRs
- Current N95 Mask Fit Test and immunizations (MMRV, TB test, or chest X-ray)
