Responsibilities will include, but are not limited to, addressing patient‑specific drug‑related problems, collaborating with prescribers in the optimization of medications, clinical review and processing of prescriber orders, supporting the Medication Reconciliation process, engaging in various quality improvement/safety initiatives or projects. The pharmacy program is committed to the learning organization culture. As such, you will act as a preceptor to both pharmacist students and interns. You will work collaboratively with other health professionals, meeting your scope of practice within the hospital setting that is guided by policy and the pharmacy practice model of the organization.QUALIFICATIONSBSc Pharmacy or PharmD degreeCurrent registration with the Ontario College of Pharmacists in part A of the registerHospital pharmacy residency required; or related clinical experience consideredDemonstrated ability to prioritize workflow and patient care activitiesFlexible,
