This position involves working collaboratively with clients and their families to establish realistic goals and implement a tailored care plan. You'll assess client needs, manage care coordination, and ensure seamless transitions of services, all while adhering to evidence-based practices. Key Responsibilities:
Partner with clients and families to define care goals
Assess and implement tailored occupational therapy plans
Coordinate care with primary and other healthcare providers




Facilitate client navigation through community resources
Work collaboratively in an interprofessional team Requirements:
Registered Occupational Therapist in BC (CHCPBC)
Experience in Home Health clinical practice
Valid BC Driver’s License for local travel
