The Community Health

Worker plays a crucial role in the Population Health Model Program, working collaboratively to implement health education initiatives and support system navigation. You will coordinate efforts with Community Health Ambassadors and partner organizations, focusing on effective community engagement and relationship building. Key Responsibilities:
