This part time role enhances community health. The Barrie & Community Family Health Team is looking for a 0.5 FTE Nurse Practitioner committed to delivering patient-centered primary health care.

You’ll engage in health promotion, assess chronic diseases, and collaborate with a multidisciplinary team to optimize care. Robust advocacy for patient health and safety is essential. Key Responsibilities:
Deliver primary health care that meets CNO standards
Implement health promotion and disease prevention strategies
Improve patient access to diagnosis and treatment
Consult with health care team members effectively
Develop and coordinate individualized patient care plans Requirements:
Valid registration with the College of Nurses of Ontario
NP-PHC program completion required
Experience working in inter-professional settings
