The Clinical

Coordinator will report to the HOCS Leader and assist at-risk communities through a variety of outreach services. With four years of related experience and two years in clinical coordination, you will implement care plans and enhance service delivery at emergency shelters. You will be an essential liaison among community agencies, health services, and management. Key Responsibilities:
Coordinate health and social service needs for marginalized populations
Provide consultation and assessment for community health services




Implement service plans in collaboration with agency staff
Adhere to privacy and documentation standards
Increase individual and organizational capacity in service delivery Requirements:
Four years of community experience, two in clinical coordination
Relevant health or human services degree required
Non-violent Crisis Intervention Training
Valid driver’s license and vehicle required
