Make a difference as a Registered Social Worker specializing in mental health for older adults (55+) in the Anishinabek community. Provide dedicated support in achieving independence, dignity, and quality of life.Your role will involve supporting Anishinabek adults and families through mental health counseling and psychosocial assessments. You will help clients navigate complex healthcare transitions while collaborating with social and health services to address their comprehensive needs.Key Responsibilities:
Facilitate care plan goals to enhance quality of life
Offer individual counseling for trauma, grief, depression, and anxiety
Conduct assessments to evaluate mental and social needs
Coordinate with healthcare services for holistic care
Help prevent caregiver burnout through education and supportRequirements:
Bachelor's level social work degree (BSW) required, MSW preferred
Must be in valuable standing with OCSWSSW
Background checks necessary
Familiarity with geriatric assessment tools
