Shape the future of community care as a full time Registered Nurse at the Population Health Clinic in East Toronto. Deliver essential health services in a cooperative, patient-focused workplace.
Reporting to the Manager at Flemingdon Health Centre, you will be midwife to transformative health experiences for residents, particularly those facing barriers. Your role will involve coordinating care, providing education, and integrating community resources to facilitate better health outcomes.
Key Responsibilities:
• Offer comprehensive care within the RN scope of practice
• Engage in client education and group health promotion
• Provide telephone triage and follow-up support
• Conduct outreach and represent the centre in community initiatives
• Participate in quality audits and team evaluations
Requirements:
• RN registered in good standing with the College of Ontario
• Minimum three years nursing experience in primary care
• Bachelor’s degree in nursing required
• Strong assessment, promotion, and counseling skills
• Experience with chronic disease management is a plus
