This role focuses on nutrition assessments, education, and interdisciplinary teamwork.To qualify, you must hold a Bachelor’s Degree in Applied Nutrition and have completed a Dietetic Internship. As a key player, you will provide nutrition support, evaluate nutritional statuses, and create care plans. Collaboration with healthcare teams and individual follow-ups will be essential in promoting optimal patient outcomes.Key Responsibilities:
Conduct health and nutrition assessments for clients
Process referrals and prioritize client needs
Monitor nutritional status and implement diabetes strategies

Maintain comprehensive records and statisticsRequirements:
Bachelor’s Degree in Applied Nutrition
Completion of Dietetic Internship is mandatory
Eligible for registration with the College of Dietitians of Ontario
Certified Diabetes Educator or willingness to obtain
